What is Assessment?
Assessment involves the collection of data from the patient, family, and other sources (i.e. medical record, other health professionals) (Potter & Perry, 2005). Data can be subjective or objective information about the patient, such as current and past health status and functional status as well as coping patterns, cultural beliefs, etc. Once obtained, the nurse will analyze the information and develop nursing diagnoses and an individual plan of care for the patient.
Subjective data- verbal cues from patient, family/relatives, client's perceptions about their health problems (feelings of anxiety, pain, or mental stress)
Objective data- concrete information; observations or measurements by data collector (vital signs, diagnostic and laboratory test results, physical examination)
The client is the best source of information. However, the setting in which the nurse interacts with the patient may impair the opportunity to obtain extensive information (Potter & Perry, 2005).
For example if the patient is in the emergency department complaining of acute symptoms, will the nurse be able to obtain full information on the patient?
If the patient is coming in to the primary care clinic for a routine checkup, will the nurse be able to obtain full information on the patient?
